How to register for a FREE screening at Mission Hills High School on September 30

Please note that you are registering for a window, not a specific appointment time. Arrive anytime within the window.

On the day of screening:
1) Bring your screening packet downloaded from Step 2.
2) Participants should dress in t-shirt and sweat pants or sports shorts. Girls wear a sports bra.
This screening does not substitute for a regular on-going relationship with a primary care physician, who is attuned to your medical history and any changes in health status. No screening can identify 100% of the individuals at risk for a sudden cardiac event. We encourage you to continue to have yearly physicals, and discuss any concerns or changes in your health with your primary physician.
IMPORTANT: Even though this is a free service to the community, it costs the EPSAL Foundation $50 per registered participant plus the commitment of 150 medical and nonmedical volunteers. We appreciate you not missing your appointment once you have registered.
