Signed Acknowledgement Form

Pupils and parents sign an acknowledgement of receipt and review of the sudden cardiac arrest symptoms information sheet provided by the school before pupils participate in an athletic activity each year.

The school shall distribute, collect and retain a copy of the information sheet for each athlete participating in CIF-governed athletic activities.

For non-CIF athletic activities, pupils and parents shall sign and return to the school an acknowledgement of receipt and review of the information sheet as posted online.

Coach Training

Requires coaches of athletic activities to complete the sudden cardiac arrest prevention training course and retake course every two years. A coach shall not be eligible to coach an athletic activity until completing the training course. Beginning July 1, 2019, a coach who does not comply with the provision to take the training course will be subject to suspension from coaching any athletic activity until completion of the required training.

Athlete Removal From Play

Requires a pupil who passes out or faints while participating in or immediately following an athletic activity, or who is known to have passed out or fainted while participating or immediately following an athletic activity, to be removed from participation by the athletic director, coach, athletic trainer or other authorized person.

A pupil exhibiting any other warning signs of sudden cardiac arrest (unexplained shortness of breath, chest pains, dizziness, racing heart, extreme fatigue) during athletic activities may be removed from participation by the athletic director or authorized person if it’s reasonably believed that it’s cardiac related.

Athlete Return to Play

Pupils removed from play must be evaluated and cleared by a physician/surgeon, nurse practitioner or physician assistant before returning to play.

Distribution of Sudden Cardiac Arrest Awareness Information

School districts and schools are encouraged to post information and materials to give pupils, parents and coaches ready access to important resources including sudden cardiac arrest warning signs, risk factors, training, safety and prevention protocol.
